$75-$90
I bill at an hourly rate ($75) for any needed court reports.
For your convenience, I can submit claims to your insurance company. I am in-network for the following:
Anthem (Blue Cross/Blue Shield)
Magellan
Sagamore
United Healthcare
I am out-of-network for the following: Aetna and Cigna. However, some insurance companies will pay a portion of the fee for out-of-network providers if the deductible is met. Please contact your insurance to verify your benefits. For Anthem (Blue Cross/Blue Shield), there is generally a co-pay and you still have to have met your deductible for services to be covered. If you do have a deductible, we can meet until the deductible is met and then sessions are covered after that point minus the co-pay.
If I do not accept your insurance, you will be given a detailed receipt called a Superbill that you can submit to attempt reimbursement on your own or apply to your HSA.
I accept checks, cash, credit cards for method of payment.
